Dear patients,

It is with regret that I must inform you that I will be closing my medical practice effective 29 December 2023 due to relocation out of province.

Your medical records will remain in the custody of Shale medical clinic. Your written authorization is required in the event you wish to have a copy of your medical record transferred to another physician at a different clinic location.
